We have passed our registration deadline of June 11, 2010. At this time all registrations will be accepted on a space available basis with full payment due at the time of enrollment.

Please download, complete, and submit the Registration Form Packet which includes all five forms listed below:

• Registration form
• Medical Activity Consent and Release form
• Declaration of Medical Insurance form
• Program Choices and Fees form
• Class Selection form
